| Missing Your Appointment |

|  The mission of Allegan Professional Health Services is to improve the health of the communities that we serve. An important factor in achieving our mission is the patient and physician relationship that exists during your treatment. If you are unable to keep your scheduled appointment it is expected that you will notify the physician office at least 24 hours in advance. To notify the office, please call 269-686-5800.
If you fail to show up for a scheduled appointment and do not notify your physician, a note will be placed in your chart indicating the time and date of the missed appointment. If you miss three appointments in a twelve month period without notifying the office, the provider may request that you be dismissed from the practice for non-compliance. If you are discharged from the practice you will receive a formal letter via certified mail. The letter will notify you of the decision to discontinue care with the physician. |
