Thank you for allowing us to assist you with your medical needs. We have been providing personalized medical care in southwestern Michigan since 1971 and we hope that our relationship with you will be a lasting one.
To save time during your first visit, please take a few minutes to complete the forms located in the “pre-registration” section of this website, or arrive for your appointment 20 minutes early to allow appropriate time to complete the forms. In addition, please bring:
- All medications you are currently taking
- Driver's license or Photo ID
- A copy of your insurance cards (if applicable)
We participate in several major insurance PPO and HMO plans in our area. As a condition of your insurance plan, any co-payments or deductibles are due at the time of your appointment. You may pay with cash, personal check, money order, Visa, or MasterCard.
If you do not have insurance, payment is due at the time of service. Our Business Office Representatives are happy to discuss payment plan options with you prior to your visit by calling our Business Office at 269-686-5888.
For your convenience, we have included directions to our practice in the “locations” section of this website. If for any reason you need to cancel or reschedule your appointment, please call us at least 24 hours prior to your scheduled appointment so that we can schedule an alternative appointment that is better for you.
